Cake is an item of soft sweet food made from a mixture of flour, fat, eggs, sugar, and other ingredients, baked and sometimes iced or decorated. I never was to Subway, so I can't tell if their bread is soft or sweet, but I believe it is not.

Well, technically molten steel can also perform these duties. It's the "technically" word that counts. Ketchup is not commonly used to perform these duties, as is molten steel, so ketchup is not an icing, nor a frosting.

Molten steel can't because it would evaporate the cake. Ketchup can make a slightly liquidy red frosting on a cake. Adds flavor? Check. Decorative? Check. I don't see the issue...?
